This file works best in Adobe Acrobat Reader — a free program you may already have on your computer. You’ll want to first download the file and save it to your computer. Then, open the file in Adobe Acrobat Reader. You may have to download it to your computer if you haven’t already.

For first-time Adobe Reader users, right-click the file and choose Open With Adobe Acrobat Reader, then you’re ready to click on a field and start typing.
